Building Bridges to Better Health (CDSM)

Building Bridges to Better Health encourages you or anyone your know living with chronic illness to take control of health and well-being by teaching the skills necessary to maintain and/or increase the activities in life that we most enjoy. Program participants will learn skills to be better able to: * solve problems and meet personal goals * talk to your doctor and make choices * handle stress and relax * eat well to live well and start a personal exercise plan * control pain and use medications effectively * deal with fatigue and frustration * and much more. 6 Week Chronic Disease Self-Management Program: Building Bridges for Better Health is a community program consisting of weekly workshops held over a six-week consecutive period in a community setting. Let's Partner! The Building Bridges for Better Health Program works in partnership with community organizations such at faith groups, hospital referred patient groups, community health centres and others who are will to host sessions in their communities across Ontario. If your organization would like to bring this program to your members or a particular group, please contact us at buildingbridges@saintelizabeth.com
