CDSM: Building up Building Bridges to Better Health

The fall 2009 session of the Building Bridges to Better Health seminars came to a successful close. We completed six seminars and all were with great “first time” community partners across Ontario. This means that we have expanded the program to six new communities in Southern Ontario. Our aim is to build health and wellness in communities through chronic disease self-management education. This program is charitable iniative supported by the Saint Elizabeth Health Care Foundation. As the demand for chronic disease self-management grows, so does our need for leaders to facilitate. Leaders are an integral part of this program. They are representatives of the program in their communities and are enablers of Building Bridges to Better Health to participants of all chronic health conditions, backgrounds, and ages. In order to continue our efforts to expand the Foundation’s Building Bridges to Better Health program to as many communities as possible, we are planning a new leader training session that will be held at the Saint Elizabeth Health Care Corporate Office the week of February 8th, 2010. We are looking for Saint Elizabeth Health Care staff to become leaders from across the organization—people who will help honour the human face of health care by giving back to their communities. Building Bridges for Better Health is also building a stronger web presence! In partnership with the Care to Know Centre, a new web space for information and collaboration is here. Be sure to check it out the stories, articles and participate in discussion forums. The program is also always looking forward to creating new community partnerships to deliver chronic disease self-management education to people living with chronic illnesses and to their caregivers. Together we can help build these stronger bridges to better health and wellness in our communities.
