How do teams use data collected using RAI/HOBIC

A topic that discusses the IP aspects of data collected using RAI and HOBIC. Are they truly collaborative assessments if only one person is inputting the data on behalf of the team; or is the assessment only representing one discipline's perspective? What would a collaborative assessment look like?

Great questions....I think it really depends on how the RAI data is inputted. I've worked in two facilities where it was completed differently. In one agency there were two nurses that were designated to completed RAIs for all of the clients admitted into acute care (~70 beds) and in the second agency each nurse was accountable to complete the RAI for the clients they were assigned to as their "primary". In each case it was only one discipline that was completing the RAI, being nursing...which certainly raises some concerns as to where the informations is coming from and from which perspective?

I recently reviewed the information nurses collect to complete RAIs with someone from our quality evaluation measures department and it was interesting to see how much duplication we create upon completion of our discipline specific assessments. I think there's an opportunity to create a holistic baseline assessment that more than one discipline can conduct collaboratively with the patient/family....and then this data can be used for completing the RAI by more than just nursing.

Interesting to hear what other think...I'm not familiar with HOBIC...
