You-Tube Video on Chronic Disease and Self-Management -- why it's important for patients to take control and be enabled to better their care and wellness. The Building Bridges to Better Health Program facilitates this by offering community workshops to caregivers of and people living with chronic illnesses. Workshops participants receive materials and training to build their self-management skills. This program is funded through community partnerships and the Saint Elizabeth Health Care Foundation.
