Did you know that the Canadian Diabetes Association recommends that people with diabetes should have 4 diabetes-focused visits with their primary care provider each year?

The following link provides a video demonstrating how comprehensive diabetes focused visits can be completed in less than 8 minutes! This video is helpful for health care providers and people with diabetes alike as an example of how to facilitate effective diabetes focused visits, and as an example of what to expect during such a visit.

One of the reasons I am so fond of this video clip, is the attention paid during the small “teachable moments” between the health care provider and the person with diabetes to diabetes education and personal goal setting. Dr. Ian Blumer, the chair of the Dissemination Committee for the CDA Clinical Practice Guidelines (and the commentator in this clip) emphasises the importance of such self management support; reminding us at the end of this video that a person living with diabetes who is knowledgeable about the key components of their own care will inevitably experience better outcomes.
