CareToKnow.org is the knowledge exchange and social networking site for the Care to Know Centre of Saint Elizabeth Health Care. CareToKnow.org is founded on a collaborative approach. We seek to create effective relationships and exchange amongst individuals, family caregivers, health professionals, researchers, organizations, policy and management stakeholders about Canadian health care.

We are a unique type of social network designed to make meaningful contributions to a body of evidence that will positively impact the health care experience of Canadians in their homes and communities. Our aim is to connect diverse audiences on CareToKnow.org — individuals/families, professionals, researchers and health care providers — so they can work together to trigger fresh ideas, identify valuable projects, facilitate unique partnerships, inform public policy and improve the point of contact between individuals and the system.

Who is the CareToKnow.org community for?

CareToKnow.org seeks to connect and create effective relationships across Canada amongst:

  • Individuals Includes individual people, family or informal caregivers, patients, home and community care clients and their families.
    Important! As an individual member of the community, there is no need for you to be an expert. You just need to have a vested interest in or desire to talk about a particular health topic or home and community care issue. We want to honour your stories and help connect
  • Professionals Includes registered nurses, registered practical nurses, personal support workers, therapists, physicians, social workers, professionals in government service, programs and policy.
  • Organizations Includes health care providers, associations, foundations; home health care, public health, primary health care, long term care, and social service organizations.
  • Researchers Includes researchers in academic and/or decision-maker settings.

Our aim is to identify and investigate new models, procedures and policies to ensure a shared approach for quality health care is achieved.

Who created the Care To Know Centre and CareToKnow.org?

The Care to Know Centre (CTKC) is the national knowledge exchange network of Saint Elizabeth Health Care. Saint Elizabeth Health Care (SEHC) has been a trusted name in Canadian health care for more than a century. Today, an award-winning organization and not-for-profit charitable leader, our dynamic talent team of more than 4,000 nurses, therapists and personal support workers deliver 3.8 million health care visits annually. CareToKnow.org features Canadian health, home and community care information and knowledge exchange, research and knowledge translation, stories about health care experiences, groups, discussions, news, resources, publications and services from:

  • Registered CareToKnow.org community members and health care professionals, public health authorities and organizations across Canada
  • Patients, families and caregivers
  • Aggregated health research, data and evidence-based analysis of the health of Canadians, to transform practice.
  • Consumer-focused applied research projects of excellence and the Centre’s own research initiatives

Do I have to register and create a profile?

Some information, such as feature articles on health topics, do not require you to login to read them. However, the Care to Know Centre is about building, engaging and facilitating conversations. Creating a profile on CareToKnow.org is key to getting the most from your experience. Registering allows you to: 

  • Create, promote and exchange information to create strong two way engagement between individuals, caregivers, researchers, organizations, policy and management stakeholders at a national level.
  • Post comments on feature articles, groups, forums, events and other content
  • Add and share information with friends
  • Create and join groups, and receive notifications of which groups colleagues and friends are joining.
  • Personalize and customize how content from the CareToKnow.org site is organized on “My Homepage”, to deliver information that is relevant to you.
  • Access and share current, quality evidence-based research, news and stories pertaining to home and community health care across Canada.
  • Participate in forum discussions.
  • Send and retrieve messages with colleagues to exchange ideas, information and opportunities.

How can sharing knowledge impact my health?

At the Care to Know Centre we are aware that there is an important topic that impacts everyone, our health. We all have had an experience we can share about our own health, care we have received and the impact that it had. It can be simple, like when a nurse provides stitches for a cut. Or it can be complex, like when researchers, health organizations and clients interact in search of new knowledge, treatments and ways to deliver quality health care. In the end, improving the health experience is about learning and sharing and we are proud to provide that exchange.

Important: It is up to you to set privacy settings for the personal health information that you share – yours, or a family member’s. You can make your content accessible to the public (available to anyone on the Internet), only registered users, or only friends within your CareToKnow.org network. 

Health care is about more than just accessing information; it is about the quality and relevance of that information. We want to know how this information can help you, what is still missing, what does it all mean to you and how can we, collectively, generate new ideas that will change the way care is delivered. What are the mission, vision and objectives of the Care To Know Centre?

Mission
The Care to Know Centre will harness the knowledge of leaders including clients, decision-makers, care providers and researchers to transform and promote a unique client-focused health care experience.

The CTKC is founded on a collaborative approach that seeks to mobilize input and to create effective linkages and exchange amongst clients, caregivers, researchers, policy and management stakeholders.

Our aim is to identify and investigate new models, procedures and policies to ensure a shared approach for quality health care is achieved.

Vision
The Care to Know Centre will act as a change catalyst of client-focused care through partnerships as well as knowledge generation and exchange initiatives that mobilize client perspectives to improve and influence health experiences, practices and policies for Canadians.

Guided by our advisory committee, we seek to implement social innovation strategies that incorporate broad experiences and evidence into effective, applied decision-making impacting an individual’s point of care through to systems approaches.
 
Objectives
The Care to Know Centre: 

  • Collaborates with clients, families, researchers and health stakeholders to identify and select essential research questions, projects and issues about health experiences.
  • Partners with clients, in Canada and around the world, in an on-going dialogue about what is needed to transform their health experience and disseminate that information to stakeholders and decision-makers to transform the experiences.
  • Funds consumer-focused applied research projects of excellence, in addition to pursuing the Centre’s own research initiatives and conducting research and consultation for others.
  • Assembles, mobilizes, disseminates and acts on evidence to transform practice and policy
